For recommendations to connect with mental health care services

• Fill out the Referral Request Form below to request an appointment

• Once we receive your request, we will contact you to set up an appointment to learn about your needs

• Based on your needs, we will try to provide you with referrals for you to research and follow up with

• Currently, REVA is able to best serve patients in California for private resources and in Santa Clara County for public resources.  However, REVA's network is growing, and we may be able to help you find care in other locations in the US.
